Online Personal Training Liability

I am interested in writing personalized programs for clients, and conducting my business online (without doing any in-person training). What are the legal implications for this?

1) How do I construct a liability waiver (and do I need a lawyer for this for it to hold any weight)?

2) Is liability insurance necessary? If so, what kinds would cover online personal training? I have had liability insurance in the past for in-person training, but it is ambiguous if online training would be covered.

3) I am considering writing a form that must be signed by a physician to show that the person is physically healthy enough to begin the exercise program. What should this form contain?
