Pregnant Woman Simulation - Hmmmmm!

I just returned from having failed my first attempt at the ACE Certification Personal Trainer Exam. The main stumbling point that threw me for a loop related to one of the two client case scenarios.
The situation called for program design and recommendations relating to a 28 year old pregnant woman six weeks into her pregnancy. The case verbiage advised that this client had in fact been cleared for exercise by her primary care physician.
I recalled from my practice test cases used to study for the exam that under no circumstances should a trainer take on a female client in the initial phases of pregnancy? I kept searching for a "refer back to physician" option to no avail.
For what it's worth, just a legitimate concern regarding the test design of the study material and answers provided versus the real thing.
